The Centre, Rolla's Health and Recreation Complex
Corporate Pass Program

The City of Rolla's health and recreation complex, The Centre, offers a discount to Missouri S&T employees on annual passes to the facility. The entire cost of the passes is the responsibility of the individual employee. Details of the program including enrollment/renewal forms follow. Summary of Program

Missouri S&T employees (full time, part time and graduate student employees, excluding temporary employees) currently can receive a 20% discount off the regular cost of annual passes. The program applies to 12-month passes only.

Annual pass holders gain use of the the Centre's, facilities as well as discounted rates for aquatic, recreation, fitness, sports programs offered at the Centre and discounts on rental of meeting rooms.

Enrollment/Renewal and Payment Process

Steps For NEW Enrollment

1. Access application form and fill in the information requested. Note: access to form requires your campus username and password.
2. Read the information on the second page of the application form and sign and date the waiver.
3. If you have chosen not to make one annual payment, determine other payment option preferred - monthly debit/credit card transaction or monthly electronic checking account deduction. If debit/credit card or monthly checking account transfer is selected, download, complete, and sign (in two places) the Direct Debit/Credit Authorization Form. Under the monthly checking account option, a voided check or copy of check (not a deposit slip) must be submitted with the forms.
4. Deliver your completed application form along with a check made payable to The Centre , or credit card information for one lump sum annual payment, or the Direct Debit/Credit Authorization Form to the front desk at The Centre, 1200 N. Holloway between 8:30 AM and 4:30 PM Monday through Friday.

Employees should allow at least one business day for The Centre to process the application. Your new membership cards will be available at the front desk upon completion of the processing.

Steps For RENEWALS

1. Access application form and fill in the information requested, however, for renewals, it is not necessary to complete the section for the names of all persons on the pass unless there will be a change from your existing list. Note: access to forms requires your campus name and password.
2. Read the information on the second page of the form and sign and date the waiver.
3. Direct Debit/Credit Authorization Form must be completed if monthly payment plan is being utilized. If you are using the same checking account as previously, you do not need to submit another voided check. Simply write "same" in the checking account information area..
4. Deliver your completed application form along with a check made payable to The Centre, or credit card information for one lump sum annual payment, or the Direct Debit/Credit Authorization Form (see #3 above) to the front desk at The Centre, 1200 N. Holloway between 8:30 AM and 4:30 PM Monday through Friday.

Employees should allow at least one business day for The Centre to process the renewal application.
